Patient Transfer

The Atlantic Health System Patient Transfer Center is a service offered to hospitals, physicians and family members who wish to transfer a patient to an Atlantic Health System hospital, including: Morristown Medical Center, Overlook Medical Center, Newton Medical Center, Chilton Medical Center and Goryeb Children’s Hospital.

This service provides one point of contact to streamline patient transfers. Our process requires a physician-to-physician conference, which our center facilitates. The Patient Transfer Center is staffed 24/7 with on duty, highly experienced, transfer nurse coordinators to ensure a smooth process.

How the Patient Transfer Center Works

When a transfer request is received the Patient Transfer Center:

  • Coordinates the transfer process via a single point of contact
  • Facilitates identification of a receiving specialist based upon the patient’s condition
  • Secures and assigns an appropriate level bed
  • Facilitates RN-to-RN report to provide a safe transfer process
  • Coordinates all transport arrangements appropriate to patient condition including specialized transports such as critical care air (helicopter and fixed wing) and ground transport, IABP (intra-aortic balloon pump), ventilator and ventriculostomy
  • Facilitates communication of patient status and provides updates to all involved clinicians
  • Coordinates with sending facility to review insurance requirements, such as eligibility and authorizations, for non-emergent cases
  • Assists in ensuring compliance with laws regarding health care access
  • Assists with out-of-country and out-of-state transfers back to an Atlantic Health System medical center for patients who are injured or ill while away from home

Accessing the Patient Transfer Center

To access Atlantic Health System’s complete line of specialty inpatient services, simply place a call to the Patient Transfer Center and provide the following information:

  • Patient’s name, diagnosis and reason for transfer
  • Sending hospital’s name and sending unit phone number
  • Sending physician’s name and contact information
  • Clinical status of the patient
  • Be prepared to fax the patient’s face sheet to the Patient Transfer Center
