Diabetes management starts with you. If you are diagnosed with diabetes, our team of experts will help you manage your disease the best way forward. Our Diabetes Education and Management Program at Atlantic Medical Group will provide you with the knowledge and support you need to stay healthy.

We offer several convenient ways to learn about diabetes self-care. You can meet with us in person at one of our locations in the greater Union County area, or if you prefer to connect with us from the comfort of your home, you can use our telehealth service.
Our multi-disciplinary team of diabetes educators, endocrinologists, social workers, pharmacists and community health experts work closely together to help patients improve their quality of life.

Experts in diabetes care are here for you. We will coordinate your care with your primary care provider. This program has been generously funded in part by The Healthcare Foundation of New Jersey as well as other funders.
Our team also consists of ambulatory care clinical pharmacists, certified diabetes care and education specialists, community health workers and licensed clinical social workers.
