This FREE in-person event will present topics including:
- Goals of care
- Heart Failure
- Chronic Obstructive Pulmonary Disease
- Pneumonia
- Diabetes
- Hospital/SNF transitions
- Behavioral Health interventions
Attendees will be able to demonstrate knowledge of pathophysiology of disease, interventions, POLST forms and goals of care, and use of respiratory equipment, all aimed at improving health of individuals in long-term care as demonstrated by nursing participants obtaining a
post-test score of >=75%.
