Home-Based Services

Whether due to a lack of access or other barriers, adults may find it increasingly difficult to get medical care as they age. That’s why Atlantic Visiting Nurse offers a free Senior Support Program that helps patients of Morristown Medical Center manage their health from the comfort of their home. Our social worker can coordinate:

  • Referrals to community-based programs 
  • Assistance scheduling medical appointments and transportation
  • Psychosocial support and counseling
  • Long-term care planning
  • Education about medications and chronic illnesses

These services are designed not only to preserve independence, but also to prevent unnecessary hospitalizations and emergency room visits.

Eligibility
The Senior Support Program is open to Morristown Medical Center patients who have a chronic condition, but don’t qualify for home care or hospice services under Medicare, or are at a high risk for frequent hospital admissions.

Home Repair and Chore Services

These programs can help seniors with home maintenance and repairs so that they may remain safe at home. 

Medical Equipment

Medical equipment includes assistive devices that can enable people to maintain their independence, such as walkers, commodes, wheelchairs, nebulizers, specialty beds or oxygen. Some equipment may be covered by insurance. It is possible to check with local ambulance corps, health care and community organizations for availability of used or donated equipment. 

  • AtHome Medical
    This company provides durable medical equipment including home oxygen, respiratory equipment, home enteral equipment, specialty beds and support surfaces.

  • Goodwill Home Medical Equipment
    This non-profit organization provides refurbished durable medical equipment to anyone, with priority given to those with limited resources for getting such equipment due to lack of health insurance or coverage of specialized equipment.
