Insurance coverage for genetic testing varies significantly by insurer. You should contact your insurance company directly to determine whether they cover genetic testing as part of your plan. When you contact your insurance company, you should have the following information:

• Name of the test
• CPT codes: These codes, which can be obtained from the laboratory that performs that test, tell the insurer what lab work will be done and how much their policy will pay for it.
• ICD-10 codes: These codes specify a medical diagnosis. Your doctor will need to provide these codes. If a letter of medical necessity is required, ICD-10 codes can be included in the letter.
